My identity was compromised by your site allowing Brandy Gill to use my information and so another Jotform was created from my own identity. I have never used your form until trying to apply for that home. She or He then created another "false" Jotform which came to my email. So where did I create another when she or he sent me the link for the Jotform to fill tf out in the first place. Stop minimizing yall involvement, no miscommunication just a sorry a** site that criminals use and you take no accountability for. Your reply is LATE And A LIE.

This platform has allowed a scammer named Brandy Gill to send a link as an application for a home on Craigslist. It seems like these two platforms helps scammers and killers but actual customers with serious issues they ignore. Someone sent me another Jotform email after I reported them and Jotform lied and stated I did another one myself. They did not admit fault for even allowing it to happen and could not provide reason as to why. DO NOT USE. They will blame you for the wrong done against you and still allowing criminals to use THEIR SITE‼ Seems like Craigslist and Jotform maybe even in on these scams since they don't reply back to complaints or reports!

Don't use these people they are worse than the actual criminals in the world! Medicaid works with these frauds to cover everything they do that violates people rights and stealing from Medicaid as whole. I guess it's better to cover up illegal activity than think people actually still do their jobs these days. This company will burn in HELL :) right along with all of you other devils and demons in this satanic world! Money won't buy you all salvation lls

To save yourself a headache and rights being violated don’t use United Healthcare nor Optima!!!
Products used:
OTC ($600) benefits were stolen by United Healthcare and Walmart. They even created false grievances to make what they did illegal seem right. Walmart stated they delivered all of what I ordered and they did not. Employees were stealing out of all the big orders I placed but mark it as if I received everything. Denied me my OTC in a check so they ordered things on my behalf I never received nor needed. These people will burn in HELL!!! :)
I contacted Craigslist to report someone stealing my identity for a property listing using their site and Jotform to make it appear official. Not a peep from these people and I will never use this damn site again for anything. Seems like people can murder and scam off this weak $#*! site and get away with it F**k you All‼You will REAP all of what you allow on your site right in HELL‼
