why have i gone to the lab ore in tacoma wa to give my insurance info to settle my bill and still get a collections notice

I hope that doesn't happen to me. They have my insurance and still said I was not identified or not eligible for benefits on the date services were rendered. Though I have been to the Dr many times before the services and no problems with my insurance. I cant even talk to a human.
